How to make a claim
Fill in the Claim Form online, or send it to us by email or by post. There is no cost, and you do not need a lawyer.
Step 1: Get the Claim Form
Fill in the Claim Form online, or download the Claim Form in a version you can fill in on your computer or one to print and fill in by hand, or call us free at 1-800-639-0225 and we will email or mail one to you.
Step 2: Fill it in
Take your time. You do not have to do it all at once, and you do not have to do it alone.
- Fill in every section that applies to you.
- Write “N/A” or “Not applicable” on any section that does not apply.
- Give as much detail as you can remember. If you cannot remember an exact date, describe roughly when it was: your grade at school, the season, or something else that was happening at the time.
- If you are not sure about something, say so. Do not guess.
- If you are filling out the form offline and need more space, type up additional pages or use extra sheets of paper. Write the question number at the top of each extra page.
Although no supporting documentation is required simply to submit a claim, if you have anything that shows you went to the school, like a report card, a transcript, or an award, please include a copy. However, you do not need to send any yearbooks.
Step 3: Sign it
The form is not complete until it is signed. You must sign to solemnly declare that the information you have provided is true and complete to the best of your knowledge, and that you understand that attempting to obtain compensation by intentionally giving false or misleading information could result in serious penalties.
Step 4: Submit it to us
You can submit your Claim Form in any of these three ways.
Online
You sign in with your email address (we email you a code each time; there is no password). Your answers are saved as you go, so you can stop and come back. Press Submit at the end and we will confirm we have received your claim.
By email
By post

If you post your form, it must be postmarked on or before the deadline.

You can also ask a family member, a friend, a counsellor or anyone you trust to help you fill in the Claim Form. However, even if someone else fills in part of the form for you, you still must sign to solemnly declare that the information you have provided is true and complete to the best of your knowledge.
