The Claim Form asks about difficult things. You do not have to fill it in alone, and you do not have to do it all at once.

Take your time

Think about when and where you fill in the form, so that you are somewhere you feel safe. You can stop and come back to it.

If thinking about what happened makes you feel anxious or unwell, that is a normal response. Please consider talking to someone you trust: a family member, a friend, a counsellor, or your doctor.

We can answer any questions you have about the form

You can call us or make an appointment. Call 1-800-639-0225 to arrange it.

You can also bring someone with you, or ask someone to help you at home. However, even if someone else fills in part of the form for you, you still must sign to solemnly declare that the information you have provided is true and complete to the best of your knowledge.

What we can and cannot do

CFM Lawyers LLP is the Claims Administrator. The Court appointed the law firm to run the claims process fairly for everyone.

We can answer any questions you have about the Claim Form, explain the process, and help you work out what documents might support your claim.

We cannot give you legal advice about your own claim. However, this claims process has been designed in a way that you should not need a lawyer to fill out the Claim Form. If you have questions, contact us.
